DAR Forms

All interactive DAR forms are housed in Emory University's REDCap system. Form submissions are saved in the system and notifications with pertinent information are sent immediately to all affected personnel. Confirmation messages are also sent out to everyone who submits a form. If you have any issues submitting a form, please contact DAR IT at darit@emory.edu.
