4PET (or PET/MR) Scan Base rate is a 2-HR standard block (includes injection, uptake time, completion of scan, data upload). Additional Room Time Base rate (per hour) will be added if ROT is beyond initial 2-HR standard block.
5Basic Safety Review provided by CSIC Medical Director for completed scans.
6Fee is waived if investigator's funded study utilizes MRI scanner concurrently with simulator.
7If MR uses contrast, the Contrast Base fee will be billed in addition to standard 2-hr block rate. 
Note: ROT = room occupancy time (start of scheduled appointment time through completion of entire scan/data upload process).
